Responsibilities

Patient Transition Management

  • Coordinate and facilitate smooth transitions from hospital or facility to home care
  • Reduce re-hospitalizations through proactive discharge planning
  • Ensure continuity and accuracy of patient care plans

Clinical Coordination

  • Assess patient eligibility for home health, palliative, or hospice services
  • Collaborate with physicians and care teams to determine appropriate levels of care
  • Support development and execution of individualized care plans

Strategic Outreach & Relationship Building

  • Develop and maintain strong referral relationships with hospitals, physicians, and healthcare providers throughout San Diego
  • Serve as a trusted clinical resource for partners and families
  • Represent Salus in the community to promote service awareness

Regulatory & Compliance Expertise

  • Maintain knowledge of Medicare, insurance guidelines, and hospice eligibility criteria
  • Ensure all admissions and referrals meet regulatory standards

Requirements

  • Active LVN or RN license in the state of California
  • Minimum 1–2 years of experience in hospice, palliative care, home health, or healthcare marketing/liaison role
  • Strong understanding of Medicare and insurance guidelines
  • Excellent communication and relationship-building skills
  • Ability to work independently in a fast-paced, field-based environment

Preferred Qualifications

  • Prior experience as a Clinical Liaison or in business development within healthcare
  • Knowledge of San Diego healthcare systems and referral networks
  • Experience in interdisciplinary care coordination

Pay

$100,000.00 - $115,000.00 per year

Schedule

Full-time with flexible scheduling based on referral and facility needs
