How You'll Contribute
- Buils and maintains referral relationships to drive appropriate admissions to the hospital
- Aid and coordinate the intake and pre-admission screening process
- On-site (typically in a referring facility) clinical judgment to determine the appropriateness of the patient for admission
- Communicates rehabilitation and facility options to patients and families
- Communicates with patients, determines admission appropriateness, and assesses the patient's expectations and requirements for participation
- Secures information relating to patients' resources and benefits
- Functions as initial contact for external case management and discharge planner personnel with payors and hospital facilities
