Clinical Liaison (CL) Acute Rehabilitation

About the role

Lifepoint Rehabilitation is seeking a Clinical Liaison to join our team at Valley View Medical Center in Fort Mohave, AZ. This position plays a crucial role in driving appropriate admissions to our acute rehab unit and ensuring patients receive the best possible care.

Responsibilities

• Builds and maintains referral relationships to drive appropriate admissions to the acute rehab unit
• Affords support and coordination of the intake and pre-admission screening process
• Performs on-site clinical judgment to determine the appropriateness of the patient for admission
• Communicates rehabilitation and facility options to patients and families
• Communicates with patients, determines admission appropriateness, and assesses the patient's expectations and requirements for participation
• Sets up information relating to patients' resources and benefits
• Functions as initial contact for external case management and discharge planner personnel with payors and hospital facilities

Requirements

• Must have Clinical Licensure
• Preferred previous marketing/sales experience
• Required previous clinical experience with demonstrated skills in clinical assessment and EMR systems
• Preferred formal sales training
• Valid driver's license
