Primary Job Responsibilities
- Act as an advocate for patients until they are safely discharged to their homes.
- Generate referrals for home health, palliative care, hospice, waiver, and private duty services by building strong relationships with case managers and social workers.
- Meet with patients and their families to ensure a seamless transition of care throughout the discharge process.
- Guide patients and their families through their options and verify eligibility for care.
Requirements
- Valid RN license in the state of Pennsylvania.
- 3+ years of experience in a clinical care coordinator, liaison, or case manager role (preferred).
- Self-motivated with strong time management and communication skills.
- Clinical knowledge of disease entities common to Home Health and Hospice.
Job Requirements
- Valid Driver's License and a reliable vehicle are required.
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
