Role Responsibilities
- Stationed daily at primary account to maximize market engagement, and referral review efforts.
- Review and update CRM Dashboard for accurate reporting via Forecast and other communication platforms.
- Respond to all incoming referrals on Careport and/or other referral platforms timely, daily, and throughout the day.
- Provide definitive answers for all incoming referrals within an hour of receiving and/or by end of day, if appropriate.
- Communicate/Collaborate with Business Development and Operations leadership to expedite any delays, barriers, and solutions in real time to expedite, definitive answers.
- Timely PCC data input including patient demographics, resident representative, POA, and Guardian information.
Qualifications
- Must possess, as a minimum, a GED, or High School Diploma, with 2 years’ experience.
- PREFERRED: Associate degree in healthcare, business or similar with 1 year experience.
- Must meet health assessment requirements regulated by the state.
- Must be capable of performing the essential functions of the job.
- Must have initiative, patience, compassion, self-motivation, and solution oriented.
