- Compensation: based on experience between $70-85k plus commissions and annual bonus eligibility
How you will make an impact
- Perform onsite clinical collection of data of the referrals to determine appropriateness for transfer to Continuing Care.
- Perform discharge planning and care coordination for residents transferring from acute hospital to Continuing Care or returning to Independent Living at the Erickson Living community.
- Collaborate with physicians regarding anticipated discharges from facilities.
- Identify quality issues and participates in quality management activities, as needed, with Administrator and/or Medical Director.
What you will need
- Minimum of 3 years of clinical experience in an acute hospital setting preferred.
- Liaison experience or healthcare marketing background preferred.
- Familiarity with acute hospitals, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market preferred.
