About the Role
The Clinical Liaison (CL) assesses potential residents/guests in hospitals to establish eligibility for care and screen for appropriate placement. This position builds and maintains key relationships with physicians, discharge planners, senior living providers, home health agencies, and other healthcare providers.
Responsibilities
- Evaluate potential patients and assess their needs in collaboration with families, referring case managers, and physicians.
- Monitor market changes and assist in developing action plans to address them.
- Participate in meeting occupancy levels aligned with the marketing plan.
- Develop positive relationships with discharge planners, physicians, families, senior living providers, and other healthcare providers to market company programs and services.
- Actively communicate with prospective residents/guests and key influencers through calls, visits, letters, and tours.
- Stay updated on operational, competitive, and company information to respond effectively to inquiries.
- Assess potential residents by reviewing medical records and collaborating with interdisciplinary teams (physicians, nurses, therapists, social workers, case managers).
- Educate potential residents, families, referral sources, and payers on program services and available resources.
- Coordinate with staff to ensure a smooth discharge process.
- Communicate company admission criteria and obtain payer information (Medicare, Managed Care, Private Pay).
- Gather current medication, treatment information, and healthcare needs of potential residents.
- Ensure accurate clinical admission records are obtained from other healthcare facilities.
- Transmit prospective resident information to the Admissions Coordinator.
- Perform additional duties as assigned.

Requirements
- Associate’s degree required.
- Knowledge of techniques to assess and treat human injuries and diseases, including symptoms, treatment alternatives, drug properties, and preventive healthcare.
- Effective verbal and written communication skills, with strong presentation abilities for diverse audiences.
- Interpersonal skills to build constructive relationships with clients, families, and customers.
- Excellent customer service skills.
- Knowledge of regulatory standards, compliance requirements, and hospital policies.
- Ability to manage territory and accounts.
- Patient care experience.
- Previous discharge planning experience preferred.
- Sales/Marketing experience preferred.
- Clean driving record.
Qualifications
- Valid driver’s license.
- RN, LPN, RRT, or paramedic certification preferred.
