About the role

Clinical Hospital Pharmacist responsible for drug therapy of assigned patients; provision of clinical drug information to meet the needs of physicians, nurses, and patients; education of pharmacy students, residents, and interns; and distribution of medications.

Responsibilities

• Evaluates medication therapy for appropriateness, considering:
  • Relevant patient conditions including age, allergies, renal status, and other disease states
  • Safety and potential efficacy of the prescribed medication (identity, strength, dose, frequency, duration of therapy, and directions of use)
  • Formulary status and availability of prescribed medication
  • Potential for drug-drug or drug-food interactions
  • Potential alternative therapy (less costly or therapeutically superior)
• Provides therapeutic drug level monitoring when appropriate
• Communicates effectively with nurses and prescribers when errors are detected or to optimize therapy
• Consults with other practitioners (other Clinical Hospital Pharmacists, Pharmacy Management, Attending Physician, etc.) as needed
• Identifies trends during routine provision of pharmaceutical care, collects supporting data, and formulates practice recommendations to improve outcomes or reduce cost
• Presents data to appropriate committees as required
• Performs formal, written consults and documents all appropriate clinical consult information in the medical record using standard hospital guidelines
• Demonstrates accuracy in dispensing, preparing, mixing, labeling products, filling automated dispensing machines, and computer data entry
• Identifies ways to decrease the possibility of medication dispensing errors
• Completes tasks with appropriate speed without sacrificing accuracy
• Assists supervisors by suggesting changes in procedures, programs, or systems to improve service, patient outcomes, customer satisfaction, reduce cost, enhance revenue, or secure JMH assets
• Develops the skills of others through orientation, active participation in staff development programs, and education/training of pharmacy students, interns, residents, nursing, medical, and other healthcare personnel
• Improves new and junior staff performance through coaching sessions
• Meets area-specific quantitative requirements and consistently receives satisfactory evaluations for the quality of tasks
• Volunteers for additional teaching assignments and consistently receives exceptional evaluations
• Evaluates and offers suggestions for continued enhancement of training programs
• Identifies the need for and initiates coaching sessions with junior pharmacists and technicians; maintains written documentation of these sessions with recommendations to area supervisor
• Makes one presentation per year at local or national meetings or has published in an internal or external publication at least every other year
• Makes one journal club and/or one in-service presentation per year

Requirements

• Pharm.D. degree and residency training or equivalent proven experience
• Possession of or eligibility for Florida pharmacist license
• Generally requires 5 to 7 years of related experience
