Clinical Director - Home Health - FT Days

About the role

The Clinical Director of Home Health oversees the delivery of evidence-based clinical patient care and ensures compliance with relevant standards and regulations. This role also serves as the Director of Professional Clinical Services (DPCS) and the Alternate Administrator for the Home Health License.

Responsibilities

• Create a culture of effective communication and foster positive relationships with internal and external stakeholders.
• Communicate policies, standards, institutional changes, safety information, and clinical updates to staff and patients.
• Delegate and direct unit activities and patient assignments, utilizing professional and respectful communication.
• Follow up on patient and family service issues within 24 business hours and conduct follow-up phone calls and field visits to enhance the patient experience.
• Operationalize the Shared Decision Making Model of nursing practice and involve point-of-care staff in decision-making processes.
• Frequent informal feedback on work performance and use coaching techniques to promote employee growth.
• Address personnel issues to maintain a healthy work environment and facilitate prompt conflict resolution among employees.
• Conduct at least ten staff meetings per year and evaluate the quality and effectiveness of nursing practice and healthcare delivery.
• Review and follow up on incident reports, report trends to staff, and analyze clinical outcomes data to improve structure, systems, or outcomes.
• Participate in multidisciplinary meetings/rounds to address and resolve patient care issues.
• Ensure payroll is submitted bi-weekly according to deadlines set by the Payroll Office.
• Manage unit activities to meet established productivity standards, such as admissions within 48 hours of referral or physician ordered start of care date.
• Facilitate and support recruitment and retention activities.
• Delegating responsibilities appropriately based on staff's licensure, education, and experience.

Department Specific Competencies

• Integrate Home Health and Hospice into the health care continuum system.
• Represent Home Health and Hospice in the community and collaborate with other institutions, public bodies, government agencies, organizations, services, groups, and individuals.
• Participate in Quality Assessment Performance Improvement (QAPI) meetings to identify areas for improvement in clinical documentation.
• Incorporate identified areas for further study, use conclusions from trended data, and recommendations from the QAPI committee to formulate an improvement plan.
• Develop and maintain administrative procedures to ensure fiscal accountability, equitable personnel practices, efficient office management, and attainment of management objectives.
• Absorb accountability for quality and fiscal control of staffing and demonstrate leadership in patient safety activities.

Qualifications

• Bachelor's degree in Nursing.
• Master's degree in Business Administration, Health Care, or a related field.
• Two years of experience in home health management.
• Three years of experience in a Medicare-certified home health agency.
• Registered Nurse License.
• National certification as Home Health Manager, OASIS, or other ANCC certification in an area of specialty within one year of hire.
