General Description
The Data Entry Clerk supports participant care in the clinic under the direct supervision of the Clinic Manager. Responsibilities include creating and maintaining electronic medical records and databases on each participant.
Responsibilities
- Enters clinic related demographic data into the EMR including participant allergies and medication status.
- Schedules all participant clinic appointments to include interval Assessments, reassessments and triage appointments for the Participant Manager and PCP.
- Collaborates with multiple members of the IDT to schedule family conferences, visitors, and new enrollment appointments.
- Schedules psychiatry appointments with the Clinic Manager and Psychiatrist.
- Schedules and distributes new enrollments, interval assessments, and reassessments for care planning within the required timeframe.
- Coordinates transportation requests for clinic appointments and holds participants unable to attend due to medical reasons.
- Maintains and distributes daily, weekly, and monthly reports to relevant personnel and departments, including reports for Infection Control and Quality Improvement for CMS submission.
- Collects and enters ER, hospitalization, and ambulance transport information into HPMS for CMS submission and billing authorization.
- Maintains a database for respite and temporary SNF/ALF placements for billing authorization.
- Answers phones, takes messages, and directs calls as needed to assist the Consult LVN.
- Cross-trains to cover for the Unit Clerk and assist with other clerical duties, such as Progress Note Clerk duties.
- Aids physicians and clinic staff in creating specialty reports as needed.
- Performs other duties as assigned by the Clinic Manager.
Qualifications / Requirements
- Minimum 1 year experience performing data entry.
- High school graduate or equivalent.
- CPR certified.

- Knowledge of medical terminology preferred.
- Proficient in adequate computer skills.
- Ability to work effectively under stress.
- Self-motivated individual who can accept supervision in an appropriate manner.
- Dependable individual with a good understanding and appreciation of the elderly.
