CLINICAL COORDINATOR, PHARMACY - VARIABLE, HFMH - PHARMACY

About the role

Location: US:WI:MANITOWOC at Froedtert Holy Family Memorial Hospital facility. This is an on‑site position with 1.0 FTE, standard 40‑hour work week.

Responsibilities

• Provide direct patient care.
• Provide leadership for clinical practice and operational efficiency within area of specialization.
• Serve as an extension of the leader to promote clinical practice advancement.
• Ensure adequate staffing based on workload.
• Resolve policy or operational issues.
• Develop and assess pharmacist competence.
• Ensure patient care needs are met by the team.
• Perform other duties as assigned.

Experience

• Minimum of one year of experience in area of specialty required.
• Three or more years of experience in area of specialty preferred.
• Previous leadership experience preferred.
• PGY1 residency preferred for Oncology Pharmacist positions.

Education

• Bachelor’s Degree in Pharmacy from an ACPE‑certified school of pharmacy or a Pharm. D. required.
• Successful completion of ASHP PGY1 Pharmacy Residency Program and/or PGY2 Specialty Pharmacy Residency Program preferred.

Licensure

• Active Wisconsin Pharmacist License required (or eligible and must obtain within 3 months).
