About the Role

The Clinical Coordinator II supervises professional social work services, including staff supervision and carrying a small caseload. This role reports to the Managing Director or Director of Clinical and Social Services and collaborates closely with the Building Manager.

Responsibilities

  • Provide clinical and case management supervision to all social work and case management staff.
  • Offer access to clinical supervision to all staff as needed.
  • Review applications for admission, participate in home visits, and contribute to admission decisions as a member of the Intake Committee.
  • Provide clinical support and resources for psychiatric emergencies.
  • Review and sign support plans, case management summaries, case notes, and psychosocial histories.
  • Ensure compliance with contract and regulatory requirements through regular chart audits.
  • Meet regularly with the Building Manager to review tenant and building issues.
  • Coordinate with the Building Manager on staff training and provide resident-related counseling to building staff.
  • Lead weekly team meetings with social work staff.
  • Facilitate monthly or quarterly meetings with the Building Manager and social work staff.
  • Provide orientation and training for new social work staff.
  • Review incidents and incident reports, submitting them to contracted agencies as required.
  • Assume a leadership role during site audits by contract agencies; write and submit required Plans of Correction.
  • Supervise social work interns as needed.
  • Develop 1:1 relationships with residents and assist them in obtaining medical, mental health, and entitlement services.
  • Help residents maintain family and social contacts and identify socialization activities.
  • Complete required forms and reports for resident discharges and/or transfers.
  • Provide crisis intervention services and visit residents in hospitals, nursing homes, or other care facilities.
  • Facilitate community building through group work, activities, and holiday celebrations.
  • Assist in recruiting and interviewing prospective social work or case management staff.
  • Help create and foster a mentorship program for case management and social work staff, including team-building activities, case conferences, and ongoing educational workshops.
  • Perform other responsibilities as assigned.

Qualifications

  • MSW required; LCSW preferred.
  • Minimum of 3 years of supervisory experience.
  • Minimum of 5+ years of experience working with older adults or individuals with a history of homelessness, chronic mental health issues, or substance abuse.
  • Proven ability to build, foster, and lead collaborative groups and workshops for social service staff across various programs.

Schedule

Monday–Friday, 9:00 AM–5:00 PM (35 hours per week).
