Clinical Coordinator and Scheduler

Key Responsibilities
- Work collaboratively with the DON and Administrator to support clinical and operational workflows
- Coordinate and track clinician schedules, visits, Start of Care, recertifications, and follow-ups
- Absorb and review clinical documentation for accuracy, completeness, and timeliness
- Support CHAP, CMS, and state compliance standards and ongoing survey readiness
- Absorb and support chart audits, quality assurance, and performance improvement activities (QAPI support)
- Track physician orders, plans of care, and ensure timely follow-through
- Communicate with nurses, therapists, aides, physicians’ offices, and office staff to ensure continuity of care
- Maintain organized and compliant clinical records in the EMR system (e.g., Axxess or similar)
- Help identify documentation or workflow issues and escalate to DON/Administrator as appropriate
- Serve as a liaison between clinical staff and administration
Qualifications
- LPN or RN license in good standing OR significant nonclinical home health operations experience
- Home health experience strongly preferred
- Strong understanding of home health workflows, documentation, and compliance requirements
- Familiarity with OASIS, Plans of Care, physician orders, and survey readiness is a strong plus
- Experience with EMR systems (Axxess or similar preferred)
- Strong organizational, time management, and multitasking skills
- High attention to detail with a strong compliance mindset
- Excellent communication skills (written and verbal)
- Comfortable working in office and collaborating closely with leadership
