About the Role

The Case Coordinator II is accountable for the clinical and financial outcome of care for an assigned case load. They function as an advanced practitioner in the roles of clinician, educator, researcher, case manager, and advocate. The role involves facilitating the care of observation and inpatients during their hospitalization by managing, coordinating, and monitoring resources to achieve optimal levels of independence and health for patients and families, clinical outcomes, utilization, service, and financial goals. The Case Coordinator II uses Continuous Improvement (CI) principles.

Responsibilities

• Act as assessor, planner, facilitator, and advocate for patient care.
• Manage and coordinate resources to achieve clinical and financial outcomes.
• Assist with staff development.
• Apply clinical expertise, observation skills, analytical thinking, and problem-solving.
• Utilize knowledge of adult education principles, research processes, change theory, and management skills.
• Perform financial analysis and group processes.

Requirements

• Active RN License.
• 5 years of appropriate clinical experience in adult Med/Surg.
• 3 years of supervisory experience preferred (e.g., charge nurse or team leader).
• Baccalaureate degree or commensurate experience; Master’s Degree preferred.
• Demonstrated knowledge and education in clinical expertise, observation skills, analytical thinking, problem-solving, verbal and written communication, adult education principles, research processes, change theory, management skills, and financial analysis.

Schedule

Day shift, authorized for 80.00 hours bi-weekly.
