About the role

The Clinical Care Manager supports patients with complex and chronic health conditions by providing care management, education, and care coordination services.

Responsibilities

• Assesses patient needs, readiness for change, and understanding of their health conditions
• Delivers individualized education and coaching using evidence-based approaches
• Empowers patients to build confidence, develop self-management strategies, adhere to treatment plans, and achieve their health goals
• Contributes to care coordination, chronic disease management, prevention, and population health initiatives
• Maintains patient confidentiality and compliance with all organizational and regulatory requirements

Requirements

• Current Vermont license as a LPN, RN, OT, OTA. PT, or PTA
