Role Overview

The Clinical Care Coordinator, aligned with the Patient Centered Medical Home (PCMH) model, primarily manages patient panels within primary care settings. They establish and maintain multiple patient relationships, coordinate care, and support healthcare delivery to ensure comprehensive, patient-focused services. The role involves engaging with caregivers, enhancing patient interactions, and promoting whole-person care.

Responsibilities

• Manage patient panels and coordinate care plans
• Establish and maintain strong patient-provider relationships
• Support primary care team operations and patient engagement
• Ensure adherence to care protocols and documentation standards

Qualifications & Skills

• Office experience including customer service, computer literacy, and familiarity with Microsoft Office
• Required: BLS certification at hire; Medical Assistant registration within 60 days
• Preferred: 2 years healthcare experience
