Clinical Care Coordinator LPN/RN for AL/MC

About the Role

We’re searching for a dynamic, hands‑on Clinical Care Coordinator to lead health and wellness operations in our Assisted Living and Memory Care units. You’ll be more than a nurse — you’ll be a mentor, problem‑solver, and advocate, ensuring every resident receives personalized, compassionate care while empowering your team to deliver their very best.

Responsibilities

• Ensure top‑quality care on every shift and step in to assist as needed.
• Lead, coach, and inspire nursing staff, medication technicians, CMAs, and caregivers.
• Train team members with engaging, real‑world learning.
• Maintain accurate charts and documentation.
• Assess, plan, and update individualized Clinical Care Plans.
• Spot and address health concerns early to help residents stay in their home longer.
• Oversee medication management, training, and audits.
• Share on‑call and Manager‑on‑Duty responsibilities.
• Assist with onboarding new team members.

Requirements

• Current RN or LPN/LVN license in good standing.
• 3+ years of relevant healthcare experience; supervisory experience a plus.
• Strong communication and leadership skills.
• Computer proficiency.
• Commitment to confidentiality and professionalism.
• Ability to pass background and drug screening.
