Role Overview The Clinical Care Coordinator supports comprehensive patient care by providing exceptional customer service, proactive outreach, and administrative assistance to care teams. Key duties include managing appointment scheduling, addressing socioeconomic barriers through resource coordination, conducting outreach for high-risk patients, and supporting office-based interventions to improve outcomes and reduce costs. Responsibilities Collaborate with clinic teams and participate in provider huddles to share patient data and improve outcomes.Coordinate preventive care, facilitate patient transitions, and resolve issues to ensure timely follow-up.Manage patient information in electronic records, schedule appointments, and connect patients to community resources.Support care interventions aimed at cost savings and health improvements. Qualifications & Skills Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) or recognized Medical Assistant certification.Proficiency in Microsoft Office, medical terminology, and excellent communication skills.Strong organizational, problem-solving, and compassionate patient navigation abilities.
