About the role

The Assessor is a master's-prepared counselor, licensed or unlicensed under the supervision of the Clinical Director. The Clinical Assessor works as part of a collaborative care team of healthcare professionals led by a Department of Health primary care provider to support adults with mental health and substance use disorders served in the medical home. The assessor provides comprehensive behavioral health assessments for adults by completing the Level of Care Utilization System for Psychiatric and Addiction Services (LOCUS) Adult Version 20, along with other agreed-upon assessment tools, to evaluate treatment needs, determine appropriate levels of care, and establish medical necessity. This includes diagnosing and assessing needs related to social determinants of health, crisis stabilization, and prevention services. This position is located at the DOH Medical Homes and reports to the Clinical Director.

Directions for Living is an essential services provider. All team members must be willing and able to provide vital support and essential services to the people served in the event of an emergency, pandemic, or other disaster situation.

Requirements

• Master’s Degree in Human Services (social work, counseling, or marriage and family therapy preferred)
• Licensed or Registered Intern status preferred
• At least two years of experience with a psychiatric population preferred
