Clinical Admissions Specialist - Full Time Days - Home/Hospice

About the role

Responsible for identifying potential admissions, processing and placing new resident/client requests, and tracking admission status. Completes activities related to authorization and determination of accepting facilities’/agency’s capabilities to provide appropriate level of care. Acts as a support person to facility/agency by completing resident/client admission-related tasks. Collaborates with colleagues and referral sources to plan, implement, and evaluate the admissions process.

Coordinates resident/client move-in process, including handling admission paperwork, insurance verifications and prior authorizations, and transportation if applicable. Ensures smooth transition from referring facility to location. Performs clinical evaluations on resident/client charts to determine appropriate medical services are offered at accepting facility/agency. Determines appropriate action for referring cases that do not meet facility/agency standards.

Visits with discharge planners, family members, and members of the interdisciplinary team to promote the location and use of services provided. Responsible for complying with all related laws, regulations, and policy/procedure. Educates healthcare team members regarding trends, external regulations, and internal policies affecting resource utilization and prior authorization.

Depending on location, enters data into Customer Referral Management (CRM) and the electronic medical record (EMR) accurately. Delivers high-quality customer service in a professional manner. Handles confidential and sensitive information. Coordinates several responsibilities simultaneously and serves as a resource to all team members.

• Visit with discharge planners, family members, and interdisciplinary team members
• Comply with all related laws, regulations, and policies/procedures
• Educate healthcare team on trends, regulations, and internal policies
• Enter data into CRM and EMR accurately
• Deliver high-quality customer service and handle confidential information
• Act as a resource and point of contact for team members

Requirements

• Appropriate education level required in accordance with state licensure
• Bachelor’s degree from an accredited school and/or program preferred, but not required
• Prior work experience in a long-term care facility preferred
• Currently holds an unencumbered license in one of the following:
  • Registered Nurse (RN)
  • Licensed Practical Nurse (LPN)
  • Occupational Therapist (OT)
  • Physical Therapist (PT)
  • Licensed Social Worker (LSW)
• If working in home-based services, can also hold an unencumbered license as:
  • Certified Occupational Therapy Assistant (COTA)
  • Physical Therapist Assistant (PTA)
• Obtains and maintains required department-specific competencies and certifications
• Valid driver’s license may be required due to necessary travel
• Basic Life Support (BLS) certification required

Pay

$29.00 - $46.50 per hour, depending on experience.

Schedule

• Full time, 40 hours per week
• Day shifts, 8 hours
• Flexible scheduling with work-life balance
• Travel within a 60-mile radius of Nisswa, MN for home health and hospice services
