Primary Responsibilities

  • Captures patient referrals from physicians and facilities.
  • Evaluates patient suitability for home health care.
  • Schedules and coordinates follow-up appointments with physicians.
  • Communicates patient reports to physicians and agencies.
  • Ensures physician orders are signed and communicated to the agency.
  • Facilitates the ordering and delivery of DME equipment.
  • Coordinates outpatient services and obtains necessary physician documentation.
  • Communicates with DON or Branch Manager about physician complaints or concerns.

Required Qualifications

  • Prior Home Health or Hospice experience.
  • Formalized clinical training from an accredited institution.
  • Licensed in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a reliable vehicle.
  • Excellent presentation, negotiation, and relationship-building skills.
  • Strong computer skills, including Microsoft Outlook and CRM software.

Pay and Benefits

Pay is based on several factors including local labor markets, education, work experience, certifications, etc. The salary for this role ranges from $72,048 to $108,072 annually based on full-time employment. Additional benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and a 401(k) contribution.
