Key Responsibilities
- Obtain referral information and coordinate all patient care services for an assigned group of patients with other members of the health care team.
- Review referrals for completeness.
- Obtain initial orders/prescription for care.
- Complete initial Welcome Call with the patient to determine medical needs, functional limitations and willingness to participate in care.
- In conjunction with pharmacy, determine the appropriate administration method for prescribed therapy.
- Document all conversations with patient/family member/provider in Specialty Pharmacy Operating System.
- Maintain accurate and complete information in the Specialty Pharmacy Business Operating System systems for assigned groups of patients including, but not limited to demographic information, medical information (height and weight, diabetes status, past medical history), nursing care provider, contact persons.
- Provide clinical support to the patient.
- Participate in patient care rounds to discuss pending referrals, current patients and clinical issues.
- Participates in yearly competencies as directed by nursing leadership.
Requirements
- Graduate of an accredited nursing program.
- Bachelor of Science in Nursing preferred.
- A registered nurse licensed, currently registered in state working in and in good standing with the Board of Nursing.
- A minimum of 5 years of experience in home care or home infusion therapy with management experience.
- Good communication skills, detail-oriented and organized.
- Marketing experience in the sales and service of home infusion services, home care or specialty injectables is highly desirable.
- Successful marketing experience and a clinical degree are also highly desirable.
- Ability to work as a team with marketing, clinical and reimbursement personnel to achieve patient satisfaction.
- Demonstrated excellence with presentation skills.
- Proficient with computers using standard Microsoft Office products.
