About the role

Prescribes, delegates and coordinates professional nursing care to pediatric patients through the use of nursing process and in accordance with hospital policy/procedure and clinical practice guidelines. Meets job-specific standards and demonstrates competency in identified skills for the assigned area.

Responsibilities

• Conducts holistic age-specific patient/family assessments upon visit according to defined standards; completes ongoing assessments/reassessments per appropriate clinical practice guidelines, protocols, and unit standards of care.
• Collects and documents data related to the patient’s physical, mental, developmental, emotional, and social condition.
• Demonstrates age-specific competency in assessment by obtaining and interpreting information in terms of patient needs, knowledge of growth and development, and understanding the range of treatment needed.
• Assesses family expectations for and involvement in the assessment and continuous care or treatment of the patient; meets families’ expectations during their clinic visit.
• Communicates plan of care to the patient/family so they can understand; identifies/assesses the need for MD involvement or a potential change in plan of care.
• Ensures documentation and evaluation of nursing actions meet JCAHO and organizational expectations (overall goal of 95% for FER, clinic problem list, signatures, pain assessment, etc.).
• Facilitates timely communication to/from patient/family and other healthcare disciplines; returns phone calls the same business day received.
• Reviews patient charts as assigned to facilitate coordination of care by assuring completion of tests, consultations, procedures, interdisciplinary follow-up, and referrals as needed for the clinic visit.
• Follows up on tests and lab results thoroughly and within established time frames; lab tests followed up within 48 hours after results obtained.
• Considers cultural, cognitive, emotional, and social influences on the patient/family’s ability and readiness to learn; documents all teaching on the FER including understanding of topics taught.
• Gathers patient information in a timely, concise, thorough manner and conveys this to the MD; assists MD in clinic to maintain good workflow and meet customer expectations for timeliness.
• Demonstrates effective delegation techniques.
• Performs special tasks and procedures within the scope of practice for an RN unique to Ambulatory Care clinics (e.g., database entry, scheduling of special procedures, assisting in special procedures, inpatient role, case management, education/teaching).
• Completes work with expertise, timeliness, organization, and detail; communicates effectively through written documentation or verbally to appropriate individuals.
• Coordinates patient care to ensure continuity of care.

Qualifications

• Education: Bachelor’s Degree from an accredited school of nursing or Associate’s Degree from an accredited school of nursing (required).
• Experience: Minimum 1 year of RN experience required. Incumbents with less than 1 year of RN experience must commit to work full time in a Nurse Residency Program. Experience with pediatric patients preferred. Previous experience with EPIC preferred.
• Licenses & Certifications:
  • Registered Nurse – State Licensure and/or Compact State Licensure (required upon hire).
  • Basic Life Support (BLS) from the American Heart Association (required upon hire).
  • Pediatric Advanced Life Support (PALS) within 180 days or PEARS within 180 days (required).
