About the role

The Nurse Navigator (Registered Nurse) supports the delivery of high-quality nursing care in alignment with established policies, procedures, and protocols, while ensuring compliance with all laws and regulations related to patient information management. This role focuses on guiding patients through their care journey, collaborating closely with an inter-professional team, and promoting patient-centered care.

Responsibilities

  • Provide professional nursing care in accordance with organizational policies, procedures, and protocols, ensuring safe and effective care delivery.
  • Communicate clearly and effectively with clinical team members, including physicians, through verbal and written documentation focused on identified healthcare needs and individualized care plans.
  • Collaborate in discharge planning needs in collaboration with physicians, nurses, and other members of the healthcare team to support safe transitions of care.
  • Support patient-centered care processes, including the use of evidence-based practices, safety initiatives, and process improvement strategies to enhance patient outcomes.
  • Ensure compliance with all laws and regulations related to patient information management, maintaining confidentiality and data integrity.
  • Contribute to continuous quality improvement by identifying opportunities to enhance care processes and participating in related initiatives.
  • Exemplify professionalism by engaging in lifelong learning, contributing to the science of nursing, and participating in professional governance activities as appropriate.
  • Share knowledge and best practices with colleagues to promote continuous learning and improve clinical practice across the team.
  • Uphold ethical, moral, and regulatory standards in all aspects of nursing practice.
  • Perform other duties as assigned within the scope of nursing practice to support patient care and departmental needs.
  • Maintain regular and reliable attendance, recognizing it as an essential function of the position.

Requirements

  • Current Registered Nurse (RN) license to practice in the state of Kansas.
  • Associate’s degree in Nursing or a nursing diploma.
  • Current Basic Life Support (BLS) for Healthcare Providers certification through the American Heart Association or the American Red Cross; BLS requirement may be waived if currently certified in Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS).
  • Demonstrated ability to provide direct patient care in a clinical setting.
  • Strong communication skills for effective verbal and written interaction with patients, families, and healthcare team members.
  • Ability to coordinate discharge planning and care transitions in collaboration with an inter-professional team.
  • Proficiency in applying evidence-based practices and safety initiatives in daily nursing practice.
  • Demonstrated interest in lifelong learning and ongoing professional development.
  • Ability to contribute to process improvement and quality initiatives within a clinical setting.
  • Strong attention to detail in clinical documentation and patient care activities.
  • Interest in participating in professional governance and contributing to best practice development.

Qualifications

  • Bachelor’s degree in Nursing (BSN) preferred.
  • Experience working with patient charts and medical records is preferred.
  • Familiarity with medical terminology is preferred.
  • Demonstrated interest in lifelong learning and ongoing professional development.
  • Ability to contribute to process improvement and quality initiatives within a clinical setting.
  • Strong attention to detail in clinical documentation and patient care activities.
  • Interest in participating in professional governance and contributing to best practice development.

Pay and Benefits

The pay range for this position is $32.00 - $45.00/hr.

Work Environment

This position is based in a clinic environment with a hybrid work arrangement, allowing work remotely two days per week and on-site three days per week. Typical work hours follow a Monday through Friday schedule, with shifts generally running from 8:00 a.m. to 4:00 p.m. or 9:00 a.m. to 5:00 p.m. The role involves working as part of an inter-professional healthcare team that values healthy people, healthy communities, and healthy futures.
