Clinic Nurse I, Primary Care, Full-Time, Days

About the role

Join a world-class academic healthcare system, UChicago Medicine, as a Clinic Nurse (RN) in the Primary Care clinic at our Hyde Park and Cottage Grove location. The Clinic Nurse in the Cottage Grove Clinic will act as our Population Health nurse, with a large portion of the job duties related to project improvement and not related to direct patient care.

Responsibilities

• Focuses on Healthy Planet tasks (non-clinic): Spirometry follow up: reaching out to schedule, curbside or indoor (phone calls/mychart) (~500 comer patients)
• MMR patients (comer peds) outreach, pend orders/adding to nurse schedule
• Lead project: abnormal results need consistent follow up; reaching out to families based on level of abnormal, keeping appointment, interpreting new levels, possible 2nd follow up
• Additional focus for cross coverage for medical advice calls and triage back up support
• Focuses on project improvements, metrics, and patient outcomes, as directed by the Medical Director
• Performs any duties involving direct patient care as assigned by the Clinical Nursing Manager nurse and directed by Nurse Practice Act and Hospital and Patient Services Sector policies and procedures, as needed
• Observes and records vital signs, assesses patient reactions and reports changes to Licensed Medical Physician
• Aids the physician with patient examinations or diagnostic procedures
• Collaborates with the Licensed Medical Physician in formulating plans of care for assigned patients
• Works with the patient care team to carry out the agreed upon plan of care
• Administers medication in accordance with policy and procedure utilizing knowledge base to assure patient safety
• Communicates all changes in the patient’s biophysical, emotional, and spiritual status to the Licensed Medical Physician
• Promotes and has knowledge of and utilizes appropriate patient care protocol relating to the physical and psychological needs of neonatal, pediatric, and adolescent patients

Requirements

• Currently licensed as a registered professional nurse in the State of Illinois
• A Bachelor's degree in Nursing or Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months
• ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program
• Two to three years’ experience as a registered nurse
• Pediatric experience as RN preferred
• Active BLS certification from the American Heart Association (AHA)

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity.
