A Clinical Nurse in our Transitional Care Unit plays a vital role in supporting patients as they move from hospital to home, ensuring continuity of care and promoting recovery.

Responsibilities

• Manage and implement comprehensive care plans for patients transitioning from hospital to home.
• Educate patients and families on post-discharge care, ensuring understanding and adherence.
• Coordinate with healthcare teams to ensure seamless continuity of care and quality services.
• Support patient recovery and promote community wellbeing through compassionate care.
