About the role
The Clinical Coordinator is a multifunctional position with aspects of care management, risk assessment, and quality assurance analysis and implementation. Key responsibilities include:
- Collaborating, facilitating, and providing structure to ensure evidence-based quality improvement.
- Managing collection and analysis of clinical outcomes data to develop clinical process improvement initiatives.
- Evaluating patient care data to ensure compliance with clinical guidelines and organizational standards.
- Responding to and investigating any misconduct, non-compliance, or patterns of risk detected.
- Overseeing post-discharge activities to ensure continuity of care as patients transition from inpatient to community.
- Identifying patients that will benefit from case management.
- Providing ongoing education and support to medical providers and nursing staff to support improvement initiatives.
- Ensuring that training and competency measurements are implemented for staff.
Qualifications
- Licensed Registered Nurse or Licensed Practice Nurse.
- Experience with EHR a must, along with basic computer skills.
- Knowledge of professional nursing theory and practice to give and evaluate patient care.
- Knowledge of organizational policies, regulations, and procedures to administer patient care.
- Knowledge of medical equipment and instruments to administer patient care.
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care.
- Ability to assess the specific age-related needs of patients.
- Skill in preparing and maintaining records, writing reports, and responding to correspondence.
- Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public.
- Ability to react calmly and effectively in emergency situations.
- Experience in Care Coordination/Case Management as well as transitional care is beneficial.
- Experience in leadership.
