About the role

The In-Patient Hematology/Oncology Clinical Social Worker (CSW) II/III intervenes with patients who have complex psychosocial needs, assists with eligibility determination for social programs and funding sources, and connects patients with community assistance from various special funds and agencies. The role includes providing crisis intervention to patients and families, coordinating and facilitating discharge planning, and collaborating with Case Managers, Nursing Staff, Medical Staff, and ancillary departments to support timely and safe discharges or continuing care within fiscal constraints.

The CSW III (LCSW) demonstrates advanced clinical knowledge and skills, participates in leadership activities such as program planning, educational programs, clinical supervision, and consultation with clinical teams, departments, and the community.

Department

UCSF Health’s Social Work department provides sensitive support to patients and families coping with illness, hospitalization, and medical treatment. Social workers act as patient advocates, offering counseling, resource navigation, and problem-solving for psychological, practical, and social concerns.

Responsibilities

  • Intervene with patients who have complex psychosocial needs.
  • Assist with eligibility determination for social programs and funding sources.
  • Coordinate community assistance from special funds and agencies.
  • Provide crisis intervention to patients and families.
  • Develop and facilitate discharge plans of care.
  • Coordinate resources and services to address psychosocial needs.
  • Collaborate with multidisciplinary teams to support timely and safe discharges or continuing care.
  • Practice within UCSF’s mission and PRIDE values.
  • Participate in department education and practice improvement activities.
  • Work in a collaborative model to optimize patient outcomes.
  • Provide equitable social work services to all patients regardless of background.
  • (CSW III only) Engage in leadership activities, including advanced program planning, educational programs, clinical supervision, and consultation.

Requirements

  • Master’s degree from a CSWE-accredited school of Social Work (required for CSW II and III).
  • Licensed in Clinical Social Work in the state of California/Board of Behavioral Sciences (required for CSW III).
  • Experience in a healthcare and/or hospital setting (preferred for CSW II; required for CSW III).
  • Knowledge of relevant community resources, medical and psychological sequelae of illness, and impact on social functioning.
  • Effective written and oral communication, diplomacy, organizational, and analytic skills.
  • Self-directed, assertive, and creative problem-solving in a high-volume environment.
  • Ability to collaborate effectively with interdisciplinary teams.
  • Basic pager, telephone, and computer skills, including internet use.
  • Flexibility to work at all UCSF Medical Center locations.
  • (CSW III only) Three years of experience in a healthcare setting (hospital setting preferred).
  • (CSW III only) Advanced clinical knowledge and leadership in program planning, education, supervision, and consultation.
