Key Responsibilities
- Manage the ElderSAFE shelter admission process, collaborating with admissions, accounting, social work, nursing, and medical teams to evaluate eligibility for temporary shelter placement
- Provide case management services, ensuring coordination of care and strict protection of client confidentiality
- Maintain and review client information in the PointClickCare (PCC) healthcare systems
- Manage the ElderSAFE helpline (Monday–Friday, 9:00 AM–5:00 PM), offering referrals and resources to community members and professionals
- Participate in care plan meetings and coordinate safe and appropriate discharge planning
- Coordinate internal and external services, including medical, legal, and social support appointments
- Arrange healthcare aides and transportation services as needed to support client access to care
- Partner with external vendors to deliver trauma-informed services to shelter clients
- Develop and implement safety plans in collaboration with the ElderSAFE team and CESLC security
- Communicate key updates, including admissions and discharges, to internal stakeholders via campus security alerts
- Provide consultative guidance and training to CESLC associates on issues related to abuse, neglect, and financial exploitation
- Support training initiatives for internal teams and external partners on elder abuse awareness and response
- Represent ElderSAFE in internal meetings, community coalitions, and professional conferences
- Maintain program data and assist in reporting and documentation
- Perform additional duties as needed
Qualifications
- Bachelor’s degree in Social Work (BSW)
- Licensed Clinical Social Worker (LCSW) strongly preferred
- Licensure in the State of Maryland (active or pending preferred)
- Minimum of two (2) years of experience providing direct services and case management to older adults in a healthcare setting, senior living community, or adult day program
- PREFERRED EXPERIENCE: Experience supporting survivors of elder abuse, domestic violence, family violence, or related crisis situations; Background working in shelter, crisis response, or trauma-informed care environments; Strong understanding of confidentiality, safety planning, and interdisciplinary collaboration
