Role and Responsibilities
- Cultivate relationships with physicians and hospitals to facilitate the best Home Care solutions for their patients.
- Monitor HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB).
- Monitor Intake Team email.
- Produce timely, accurate data entry.
- Enter orthopedic prescreens into HCHB according to agency protocol.
- Provide phone support to the department and triage of calls to Intake staff.
- Provide excellent customer service to internal and external customers.
- Participate in department and agency meetings.
Qualifications and Requirements
- Minimum of 4 years experience in a clinical setting; medical office experience preferred.
- Exceptional customer service and communication skills required.
- Must be proficient and efficient working with a computer; experience with EMR, and MS Office products required.
- Detail oriented.
- Accustomed to working in a fast-paced environment.
Education Requirements
Graduate of an accredited program of practical nursing required along with valid and unencumbered LPN license issued by NYS dept of education.
Work Environment
The Client Relationship Specialist is primarily in an office setting and may be exposed to outdoor conditions. The working conditions are classified as sedentary work: Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
