About the Role
Accompany persons served on medical appointments and advocate for appropriate medical services. Provide direct care services and act as a liaison between the residence and medical facilities, hospitals, laboratories, and pharmacies. Ensure that medical, dental, or other therapeutic appointments are carried out as required. Computer proficiency is required.
Responsibilities
- Collaborate with the Residence Nurse and Residence Manager to ensure proper medical follow-up for all persons served.
- Accompany persons served to obtain medical and clinic care according to their needs.
- Advocate for persons served with physicians, lab technicians, psychiatrists, dentists, and other medical service providers.
- Ensure clinical consults are completed appropriately before and after medical appointments.
- Safely operate agency vans and/or vehicles as per the agency driving policy.
- Communicate all immediate medical concerns to the Residence Manager and/or Residence Nurse.
- Participate in weekly consult review meetings with the Residence Manager and Residence Nurse.
- Obtain and file all medical documentation, including blood work, EKG, X-ray reports, and other medical interventions in the persons' binders.
- Understand, articulate, and document specific medical information.
- Assist in training persons served in activities of daily living (ADL) and development of self-help and social skills.
- Provide total support for persons served requiring assistance with diaper changing, ambulation, G-Tube feeding, transferring from bed to chair using Hoyer lift, showering, shaving, toileting, hand washing, and mouth care.
- Implement goals designed to maximize each person’s level of independence, including budgeting, grooming, nutritional planning, use of public transportation, community resources, shopping, and housekeeping.
- Display proficiency in entering person-served data documentation in the Electronic Health Record (EHR) system.
- Account for and oversee all medication as per WellLife Network’s Medication Administration Policy, including the use of the computerized EMAR (Electronic Medication Administration Record) system.
- Communicate relevant information to other staff or supervisors verbally and in writing, completing appropriate logs and incident reports.
- Physically intervene with persons served when necessary to prevent injury to self or others, assist in responding to dangerous situations, and administer emergency first aid.
- Successfully complete and perform all techniques required in AMAP, SCIP, CPR, and First Aid.
- Maintain confidentiality at all times.
- Perform other duties as requested based on the needs of the facility and/or agency.
Requirements
- High School diploma or equivalent required; Associate’s degree in Human Services preferred.
- Experience working with the developmentally disabled and/or experience in the medical field.
- Valid NYS Driver’s license is required.
