Position Summary
Accompany persons served on medical appointments and advocate for the appropriate medical services for persons served. Provide direct care services to persons served in addition to acting as a liaison between the residence and medical facilities, hospitals, laboratories, and pharmacies. Ensure that the person's medical, dental, or any other therapeutic appointments are carried out as required.
Essential Accountabilities
- Works in collaboration with the Residence Nurse and Residence Manager to ensure proper medical follow-up occurs for all persons served.
- Accompanies persons served in obtaining medical and clinic care according to the person’s need.
- Advocates for persons served needs with physicians, lab technicians, psychiatrists, dentists, and other providers of medical services.
- Safely operates agency vans and/or vehicles as per the agency driving policy.
- Communicates all immediate medical concerns to the Residence Manager and/or Residence Nurse.
- Participates in weekly consult review meetings with the Residence Manager and Residence Nurse.
- Ensures any and all medical documentation ordered and/or medical interventions, such as blood work, EKG, X-ray reports, etc., are obtained and placed in the person’s binders for all medical consultations/hospitalizations and/or further placements.
- Maintains confidentiality at all times.
- Other duties as requested based on the needs of the facility and/or agency.
Qualifications
- High School diploma or equivalent required; Associate’s degree in Human Services preferred.
- Experience working with the developmentally disabled and/or experience in the medical field.
- A valid NYS Drivers license is required.
