Client Care Navigator - CCBHC Operations

About the role

The Client Care Navigator within a Certified Community Behavioral Health Clinic (CCBHC) serves as the primary contact for clients with mental health and/or substance use disorders, including complex and co-occurring conditions. Responsibilities include outreach, screening, triage, care coordination, and facilitating access to a full range of services such as crisis care, outpatient treatment, and community supports. The role involves collaborating with providers, clinicians, support staff, and community partners to ensure seamless transitions, advocate for client needs, and promote coordinated care from intake through ongoing treatment.

Responsibilities

• Serve as the main contact for referrals and clients entering or receiving services.
• Conduct initial screenings, needs assessments, and schedule evaluations.
• Support access to care in line with standards, verify Medicaid, and maintain client records.
• Coordinate services across mental health, substance use, primary care, and crisis supports.
• Facilitate referrals, care transitions, and connect clients to community resources like housing and employment.
• Advocate for clients, educate families, promote recovery, and ensure confidentiality and compliance.
• Participate in team meetings, communicate client updates, and document accurately in EHR.

Qualifications

• A bachelor's in Social Work, Psychology, Counseling, or related field.
• 2‑5 years’ experience in behavioral health, case management, or care coordination.
