Client Care Coordinator

About the role

Home Instead is seeking a Client Care Coordinator to join their dedicated team.

Responsibilities

• Build strong, lasting relationships with clients and their families
• Travel throughout the service territory to meet new clients, conduct in-home consultations, and sign clients up for services
• Create and oversee personalized care plans to ensure every client's needs are met
• Conduct ongoing visits to monitor satisfaction and quality of care
• Communicate and collaborate with Care Professionals to deliver top-quality service
• Address client or caregiver concerns promptly and compassionately
• Participate in the on-call rotation to support clients and caregivers outside of normal business hours

What We're Looking For

• Excellent communication and organizational skills
• A caring, customer-service mindset
• Experience in caregiving, healthcare, or client relations (preferred)
• Ability to multitask and travel within the service area as needed
• Dependable, compassionate, and professional demeanor
• Flexibility to assist with after-hours calls when scheduled
