Client Care Coordinator

Responsibilities

  • Serve as the primary point of contact for new and existing clients
  • Conduct client intakes, assessments, and ongoing check-ins
  • Cookier schedules and match caregivers to clients based on needs and skillsets
  • Support caregivers with updates, training reminders, and problem-solving
  • Maintain accurate client records and documentation
  • Assist in resolving scheduling challenges or client concerns in a timely, professional manner
  • Collaborate with the office team to ensure smooth operations and excellent service

Requirements

  • Strong communication and customer service skills
  • Excellent organization and attention to detail
  • Ability to multitask in a fast-paced environment
  • Empathy, patience, and a genuine desire to serve seniors and their families
  • Previous home care or healthcare coordination experience a plus
  • Proficiency with scheduling software is helpful (training provided)
