City Connects School Site Coordinator

About the Role

Responsible to both the Principal and the City Connects Supervisor. The School Counselors, Adjustment Counselors, and City Connects Coordinators will work with the principal to schedule an annual presentation to the school staff and administrators about the school’s student support services, including each of their roles and responsibilities and the referral process for all staff to use.

Responsibilities

• If chosen by the principal, lead/facilitate a Student Support team (STAT) of multiple professionals to review the intensive needs of referred students and create actionable plans to address those needs.
• With support from the principal, facilitate a yearly discussion with each classroom teacher to assess the holistic strengths and needs of every student across developmental domains: academics, social/emotional/behavioral development, physical health, and family. This discussion results in individualized student support plans.
• Connect students to a range of prevention, early intervention, and intensive services programs addressing physical, emotional, cognitive, and social development.
• Develop and enhance community partnerships with varied agencies to meet the identified needs of students, families, and the school.
• Collaborate with school staff to implement the City Connects model.
• Engage families and caregivers in understanding and supporting their children’s education, emphasizing how academic outcomes are enhanced by addressing social/emotional and personal development needs.
• Collect data on the implementation of the model and the delivery of services.
• Participate in professional development sessions to implement the City Connects model.
• Pursue continuous professional growth.
• Perform other related duties as assigned.

Requirements

• Master’s degree in School Counseling, Mental Health Counseling, Social Work, or a related field (e.g., Psychology).
• Evidence of culturally competent practices.
• Ability to work collaboratively with a diverse staff, student body, families, and a team of professionals at both the school and university levels.

Preferred Qualifications

• Experience working in a school environment, particularly in urban public schools.
• Fluency in a second language.
• Experience with organizations that use data to guide practice.
• Licensure as a School Counselor, Licensed Mental Health Counselor (LMHC), or Licensed Independent Clinical Social Worker (LCSW/LICSW).
