CHW Wellness Navigator

About the role

As a Substance Use Navigator, you will interact and work collaboratively with the healthcare team and directly with patients in both the hospital and Emergency Department settings. Your primary focus will be bridging the gap between inpatient care and effective, ongoing substance use treatment.

Responsibilities

• Review patient census in the hospital and Emergency Department to determine patients eligible for drug intervention.
• Interact with patients in crisis and develop trust relationships to connect them to inpatient and outpatient addiction resources and care.
• Interact with multiple staff members to obtain patient history.
• Review cases with providers in multiple areas of the hospital and self-determine workload and schedule of interactions/visits.
• Refer patients to outside resources, interact with outpatient clinics, and develop positive working relationships to improve patient care and follow-up.
• Connect patients to resources such as transportation, inpatient and outpatient drug treatment, and pharmacy to obtain needed medications for withdrawal.

Requirements

• 1 year of experience in a paid or unpaid role as a Community Health Worker.
• Lived experience—personal or loved one—impacted by SUD or other behavioral health challenges, social inequity, or violence.
• CHW Certification or commitment to attain CHW training and certification within 18 months of employment.

Qualifications

• Associates or Bachelors Degree (preferred).
• Knowledge and ability to work with individuals impacted by adverse conditions, including substance use disorders, mental health challenges, justice involvement, and housing instability.
• Possess a nonjudgmental, energetic, positive approach to assisting patients with Substance Use Disorder (SUD) and Mental Illness.

Pay

$25.30 - $35.74 per hour.
