Responsibilities

• Serve as a trusted resource for families, providers, and community partners
• Navigate eligibility requirements, documentation, and program services
• Deliver high-quality case management from application through ongoing support
• Build strong relationships
• Conduct accurate income and eligibility assessments
• Maintain detailed records
• Connect families to valuable community resources
• Ensure timely communication
• Maintain data integrity
• Comply with program standards

Requirements

• Education: High School Diploma - Required
• Experience: Bachelor's Degree - Preferred
• Experience: 1-3 years - Supporting families
• Experience: 1-3 years - Case management experience, customer service experience, experience working within a non-profit, experience administering benefits

Qualifications

• Bilingual proficiency in English and Spanish preferred
