Child/Adolescent LICSW/LMHC

About the role

At CHP, our mission is to provide access to health care for everyone. This role is part of a hybrid team, working 3 days a week in-office at Berkshire Pediatrics in Pittsfield, MA.

Responsibilities

• Collaborate with all members of the healthcare team in development of the Plan of Treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
• Develop and maintain thorough knowledge of referral resources throughout the service area.
• Adhere to all third party payer requirements.
• Maintain accurate and timely records, including clinical progress notes, according to facility policy.
• Provide positive customer relations in dealing with patients, families, primary care providers and other members of the healthcare team, referral sources, services agencies and others.
• Stay current on relevant therapeutic interventions, research, and trends in the field through use of Continuing Education time.
• Adhere to all department/facility policies, including attending clinical supervisions and case consultations.

Qualifications

• Master’s degree in Social Work
• LICSW or LMHC professional license per MA state law
• One (1) year full time experience
