Schedule
- 12-hour shifts (7 AM–7 PM)
- Day and night shifts available
Responsibilities
- Operate under the scope of practice defined by the state Nurse Practice Act.
- Direct resident care and nursing staff during assigned shift, including staff assignments, mentoring, and education.
- Conduct daily nursing functions in accordance with company, state, federal, and local rules and regulations.
- Admit, transfer, and discharge residents with thorough documentation.
- Orient new residents and family members to the facility.
- Participate in facility surveys (inspections).
- Ensure high-quality, positive clinical outcomes and resident/family satisfaction during shift.
- Serve as a resource to staff regarding quality of services provided.
- Administer medications and treatments timely per facility policy; transcribe and carry out physician orders as allowed by Nurse Practice Act.
- Manage nursing personnel to limit overtime and agency use.
- Assess nursing staff performance against unit standards and goals.
- Conduct unit huddles to promote communication and provide updates to the Director of Nursing (DON).
- Facilitate shift-to-shift communication between incoming and outgoing nursing staff.
- Coordinate shift assignments, resident assignments, and rest/meal breaks for Certified Nurse’s Aides.
- Collaborate with other departments to provide timely, effective care aligned with residents’ needs and preferences.
- Assume administrative responsibilities for weekend and holiday coverage as requested.
- Stay beyond scheduled shift to meet state staffing requirements or resident needs.
- Ensure accurate and timely compliance of the MDS process reflecting the resident’s plan of care.
- Utilize Point Click Care (PCC) according to company process.
- Follow physician orders as prescribed.
- Manage medication supplies and ensure handling aligns with company policy.
- Promptly notify resident’s attending physician and family of significant health changes.
- Report incidents, accidents, and adverse events to Administrator and DON.
- Document, investigate, and address resident/family complaints, accidents, adverse events, and grievances per company policies and regulations.
- Demonstrate knowledge and appropriate use of the Company Infection Control Manual.
- Evaluate care intervention effectiveness, identify problems/trends, and develop alternative interventions.
- Perform other duties as assigned or needed.
Requirements
- Graduate of an approved Licensed Vocational Nursing program and licensed in the state of practice (New Mexico).
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
- Current BLS/CPR certification.
