Chaplain - Main Palliative Care - PRN

Responsibilities

• Provide pastoral care to referred patients/families and to staff/physicians.
• Assess for anticipatory or complicated grief.
• Provide in-person and tele-chaplaincy spiritual care to patients/families.
• Provide bereavement care for families.
• Collaborate with palliative care team:
• Provide spiritual assessment on each referred patient and family.
• Facilitate completion of advance directives (living will and power of attorney for health care).
• Attend and contribute to team rounding and conferencing.
• Aid in educational presentations to patients, families, staff, physicians, and community care providers.
• Support other bereavement support resources and spiritual care providers.

Qualifications

• Bachelor’s degree required, master’s degree preferred.
• Four clinical pastoral education units.
• Palliative care continuing education.
• Five years of experience in health care chaplaincy preferred.
• Knowledge and skills in clinical pastoral care with an emphasis in palliative and bereavement care.
• Board certification for Chaplains from the Association of Professional Chaplains preferred.
