About the role

WellSpan is proud to partner with YADAC to expand the Warm Hand-Off (WHO) Program, an innovative initiative designed to connect individuals experiencing substance use challenges with the support and treatment they need. The WHO Program provides immediate outreach, engagement, and care coordination for individuals following an overdose, substance use-related emergency department visit, or other critical intervention point in York and Adams Counties. By meeting individuals where they are and offering compassionate support, our teams help bridge the gap between crisis and recovery.

We are seeking passionate professionals who are committed to making a difference in the lives of individuals and families affected by substance use disorders. Team members will have the opportunity to work alongside hospitals, community partners, and recovery resources to ensure people receive the right care at the right time.

General Summary: Contributes in a peer support capacity to facilitate recovery and enhance wellness by sharing personal experience, practical knowledge, and lived expertise with clients and interdisciplinary teams. Serves as a role model for recovery for both staff and peers and partners with individuals in developing recovery tools, achieving recovery goals, and building support networks. Provides timely engagement and recovery support to individuals following substance use-related crises, which may include oversight of the Warm Hand-Off Program to promote treatment linkage, continuity of care, and successful recovery outcomes. Has two primary functions: direct client care and supervisory activities for monitoring assigned certified recovery specialist's work and outcomes.
