Certified Recovery Mentor (Integrated Team Based Care))

This position is based within the urgent walk-in clinic at our Garlington Health Center, located in NE Portland, OR.

Monday, Wednesday, and Friday, 8:30 a.m. to 5:00 p.m.

About the Role

The Certified Recovery Mentor (CRM) will work as a member of an interdisciplinary team to provide peer recovery support to individuals with substance abuse and mental health disorders. This position will advocate for the Individuals We Serve (IWS) and support them to pursue their personal goals and individual wellness journey. The incumbent must understand, appreciate, and respect the differences within our Cascadia community, promote integrated care, and uphold trauma-informed and person-first approaches.

Responsibilities

• Participate in clinical case conferences and staff meetings to enhance knowledge, align with program goals, and advocate for individual needs.
• Serve as an advocate within the organization and with external partners or service providers.
• Demonstrate a recovery-focused and healthy lifestyle.
• Share personal recovery experiences with IWS when appropriate and supportive.
• Ensure that the needs and voices of IWS being served are represented.
• Link IWS to various recovery support models and community resources.
• Assist IWS in developing a strong, sustainable support network.
• Provide guidance on accessing and navigating community systems such as recovery communities, housing resources, and public transportation.
• Apply Feedback Informed Treatment (FIT) practices as part of the service delivery team.
• Comply with and implement regulations, policies, and procedures under which the department and program operate.
• Assist in crisis situations as appropriate, following emergency protocols and procedures.
• Understand fire regulations and evacuation procedures; participate in regular safety drills.
• Document and maintain records in compliance with agency policies and local, state, and federal regulations.
• Maintain accurate and up-to-date documentation and complete electronic medical records (EMR) using Cascadia software.
• Adhere to mandatory abuse reporting laws, HIPAA, and 42CFR regulations.
• Complete annual employee training requirements on a timely basis.
• Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.
• Perform other duties as assigned.

Requirements

• A completed Recovery Mentor training approved by the Oregon Health Authority (OHA).
• A Traditional Healthcare Worker (THW) certification, which must be kept current and in compliance with the OAR’s.
• Experience as a former or current user of behavioral health services.
• Must self-disclose as being in active recovery from substance addiction and have personal experience with recovery services.
• Possess a valid driver’s license, one-year driving history, clean driving record, and ability to pass a Driver Motor Vehicle (DMV) check.
• Ability to perform CPR and properly use AED equipment (training provided).

Working Conditions

• This role is centered on direct IWS and community engagement and is designed for in-person work; no hybrid or remote options.
• Work is performed in administrative offices, clinical offices, acute care settings, and community settings including IWS residences, correctional facilities, shelters, hospitals, public places, and other social service sites.
• Meal periods and breaks are subject to interruption.
• Universal Precautions and environmental awareness are critical due to potential exposure to aggressive behavior, disease, biohazards, noise, and contaminants.
• The work involves addressing new and unusual circumstances, requiring flexibility and sound judgment.

Pay

Starting rate range: $23.57 - $24.37 per hour.
