Purpose Of Position

The Medication Therapy Management (MTM) Pharmacy Technician supports pharmacists in delivering comprehensive medication management services to patients. This role is responsible for patient outreach, medication reconciliation, documentation, adherence monitoring, and coordination with healthcare providers, insurance plans, and pharmacy staff to improve medication outcomes and support quality initiatives. The MTM Pharmacy Technician plays a key role in ensuring timely completion of MTM services while maintaining compliance with HIPAA regulations, Medicare Part D requirements, and company policies.

Description Of Required Duties And Tasks

  • Patient Identification & Scheduling: Identify patients who qualify for Medication Therapy Management services and schedule appointments with providers and residents for medication reviews.
  • Medication Reconciliation & History: Gather comprehensive medication lists, including prescriptions and OTC products, through patient interviews and provider records.
  • Patient Engagement & Adherence: Contact patients to review medication usage, discuss adherence, and address potential barriers to taking medication.
  • Documentation & Data Entry: Document medication therapy problems, interventions, and patient conversations in MTM software (e.g., OutcomesMTM) for pharmacist review.
  • Follow-up Coordination: Perform follow-up calls to patients regarding therapy changes, lab tests (e.g., for anticoagulation), or medication refill.

Required Knowledge

  • Knowledge of the LTC pharmacy industry
  • Knowledge of Medicare Regulations
  • CMS and other state/federal regulations

Required Work Experience

  • Previous Long Term Care Pharmacy experience
  • One to three years of pharmacy technician experience

Required Computer Skills

  • Proficiency with information systems
  • Microsoft Office

Physical Requirements

  • Occasional lifting of objects up to 50 pounds
  • Subject to standing, walking, sitting, bending, reaching, kneeling, pushing and pulling
  • Work is performed in a standard office environment
  • Must have reliable transportation

Certification Requirements

  • Active Tennessee Board of Pharmacy Registration
  • Pharmacy Technician Certification (CPhT) preferred
  • Must complete continuing education in pharmacy-related topics as required to maintain certification

Education Requirements

  • High school diploma or general education degree (GED)
  • Completion of pharmacy technician program preferred
  • Or equivalent combination of education and/or experience

  • Take reasonable and prudent actions to prevent others from engaging in unsafe practices
