About the role
The Certified Peer Supports Specialist at Hope Network is responsible for providing flexible services aimed at supporting individuals served. This role requires home visits in Macomb County and involves direct or indirect support, leveraging personal experience with mental illness to offer unique insights.
Responsibilities
- Under direct supervision, functions as part of a team to support individuals served.
- Provides expertise based on personal experience, acting as a consultant to other team members.
- Acts as a positive role model and coach for others, promoting recovery-based practices.
- Engages individuals-served and monitors their wellbeing.
- Participates in the Family/Guardian/Friends Liaison Program, fostering a supportive network.
- Builds community relations and social networks to provide support and hope.
- Facilitates access to community resources and social support networks.
- Documents progress towards goals of individuals-served at the end of each shift.
- Ensures timely submission of required internal data reports.
- Knowledgeable of contract requirements and the Program Fidelity Model.
Requirements
- Must be certified as a Certified Peer Support Specialist.
- Must complete and achieve ACT 101 training by DCH.
- Credentialing & Privileging includes General ACT, Group Therapy, Co-occurring Treatment, and Peer Services Specialist Conference.
- Willingness to become a Certified Peer Support Specialist if not already achieved.
- Two years of relevant experience preferred.
- Firsthand knowledge of characteristics and conditions experienced by individuals served.
- Concepts of Motivational Interviewing, Integrated Dual Diagnosis Treatment, Family Psycho Education, Seeking Safety, Family Psycho Education, Stages of Change, Trauma Informed Care, and Integrated Care.
