About the role
Works with enrolled parents to assist them in creating and building connections in order to help them become self-sufficient. Provides case management and outreach services, assesses clients to identify family strengths, challenges, and needs, and collaborates with community partners. Facilitates parent-led support groups and parent leadership meetings, and engages parents in Community Action Network (CAN) meetings.
Responsibilities
- Demonstrate cultural awareness and sensitivity in working with families from diverse backgrounds.
- Create and build connections with persons served to assist them in meeting their needs and becoming self-sufficient.
- Provide specialized case management and outreach services to individuals with perinatal mental health issues and substance use disorders.
- Assess persons served and family strengths, challenges, and needs.
- Collaborate and build connections with community partners.
- Assist and facilitate parent-led support groups and parent leadership meetings.
- Engage parents through lived experience and support their contributions to the CAN.
- Attend meetings promptly and actively participate in trainings, staff meetings, and clinical supervision.
- Complete documentation in a timely and accurate manner.
- Complete all other tasks assigned by the Program Manager within given timeframes.
Requirements
- High school diploma or GED required.
- CPRS Certification is required.
- Bilingual preferred.
Skills
- Familiarity with community resources, both formal and informal.
- Effective communication via written, verbal, in-person, and virtual methods.
- Ability to work in a fast-paced environment with strong time-management and multitasking skills.
- Strong interpersonal skills and attention to detail when speaking with clients and referral sources.
- Proficiency in Microsoft Word, Microsoft Excel, and electronic health record systems.

Pay
$16.50–$23.10 per hour
