Cares for residents under the direction and supervision of a registered nurse or a licensed practical/vocational nurse.

Responsibilities

  • Provide direct resident care, including assistance with bathing, grooming, dressing, toileting, incontinent care as applicable, oral hygiene and feeding.
  • Measure and record vital signs, weight, percentage of meal consumed, intake/output as directed.
  • Assist residents with mobility, transfers, and positioning using proper body mechanics and lift equipment as applicable.
  • Respond promptly to call lights, resident needs, and changes in condition; report observations to licensed nursing staff.
  • Support residents’ independence and dignity, while ensuring safety and comfort.
  • Maintain accurate and timely documentation in accordance with facility procedures.
  • Follow infection control, safety, and universal precaution guidelines at all times.
  • Provide emotional support and companionship to residents, fostering a positive living environment.
  • Maintain a clean, safe, and comfortable environment.
  • Provide postmortem care.
  • Make routine rounds on each assigned resident every two hours.
  • Provide skin care and observe residents’ skin when giving care; report changes to licensed nurse.
  • Modify own behavior in response to residents’ behaviors.
  • Be aware of developmental tasks associated with the aging process.
  • Contribute to the residents’ assessment (MDS/CAA’s) and the plan of care.
  • Provide nursing rehabilitation/restorative nursing services.
  • Promote residents’ ability to adapt and adjust to living as independently as possible.
  • Assist the resident in self-care according to the resident’s abilities.
  • Assist residents in the use of assistive devices for transferring, walking and mobility, dressing and grooming, eating and swallowing.
  • Maintain range of motion, proper turning and positioning in bed and chair.
  • Assist with bowel and bladder retraining.
  • Instruct the resident on the care and use of prosthetic and orthotic devices.
  • Incorporate training and skill practice activities, which include repetition, physical or verbal cueing, and task segmentation.
  • Care for residents with Dementia:
    • Know how to respond to residents’ behaviors.
    • Allow residents to make personal choices.
    • Provide and reinforce other behavior consistent with residents’ dignity.
    • Use the resident’s families as a source of emotional support for the resident.
    • Demonstrate a basic understanding of behavior and functional symptoms associated with dementia by providing interventions and treatments to help reduce these symptoms.
    • Use modification in the presentation of activities of daily living to maintain and maximize resident functions.
    • Use effective intervention techniques to address behavioral concerns.
    • Assist with transporting residents to activities.
    • Demonstrate effective communication techniques for residents with dementia.
  • Provide non-resident care including but not limited to:
    • Wash wheelchairs.
    • Clean soiled utility areas.
    • Clean shower and whirlpool rooms.
    • Clean kitchen areas on the nursing unit.
  • Perform other duties as assigned.

Requirements

  • High School Graduate/GED.
  • Must be a Certified Nursing Assistant in good standing with the certification board or a graduate of a state-approved nursing assistant training program and passed a state-approved competency examination (written and skills) within 120 days of employment; name entered into state nurse aide registry.
  • Computer skills are required.
  • Meets all state and federal criminal background check requirements.
