Responsibilities
- Care for seniors and individuals with illnesses, injuries, or chronic diseases in their homes.
- Assist with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating, and nutritional intake.
- Provide assistance with ambulation, transfers, and range of motion exercises.
- Monitor vital signs, measure intake and output, check blood glucose levels, and document any changes.
- Collaborate with a team of home health nurses and aides to ensure adherence to the patient's care plan.
- Observe patients and report any issues requiring adjustments in care, medications, or medical equipment to the supervisor.
Requirements
- Hold a high school diploma (or equivalent) and be certified as a Nursing Assistant.
- Possess a valid CPR certification.
- Have a valid State driver’s license, auto insurance, and reliable transportation.
- Demonstrate a compassionate nature, effective communication skills, and the ability to lift up to 50 lbs.
