About the role

Daymark is looking for candidates with a Certified Nursing Assistant background for their "Health Partner" role within their Care Team. Health Partners are core members of the Daymark care team, supporting integrated, whole-person care for our patients.

Responsibilities

  • Conduct structured social needs assessments (SDOH) and connect patients with community resources, which may include transportation, financial assistance, food, medical equipment, or emotional support programs.
  • Support development of patients’ care plans and participate in case conferences to ensure quality care delivery.
  • Serve as the primary point of contact for virtual visit readiness and basic technology troubleshooting.
  • Cookordinate and schedule care for patients, including scheduling follow-up visits and facilitating referrals.
  • Own structured chart preparation to ensure providers are visit-ready, including accurate contact, insurance, and care team information.
  • Track completion of our Symptom Survey, complete all appropriate follow ups, and escalate clinical concerns as needed.
  • Document social interventions, tasks, and care coordination activities in the electronic health record in a timely and standardized format.

Requirements

  • At least 3 years of experience as a Medical Assistant, CNA, EMT, or in a similar clinical or care coordination setting.
  • Oncology experience is a bonus.
  • You're comfortable driving to visit patients in the field.
  • Comfortable with technology, using tools like Slack, Zoom, the Google Suite, and EHR systems.
  • Bias to action: comfortable identifying and prioritizing needs and acting on them immediately.
  • Comfortable with change and ambiguity, able to figure out what needs to be done and take action.
  • Strong relational skills, able to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
  • Organized & communicative, proficient in collecting patient clinical and demographic data and documenting appropriately in a timely manner.
  • Open to working from Daymark's office: comfortable working out of their office in Providence, Rhode Island 5 days a week, and visiting patient's homes whenever needed.

Qualifications

  • Bilingual: a plus but not required.

  • Effective communication and documentation skills.
  • Ability to manage multiple tasks and prioritize responsibilities.

Pay

$50,000 - $60,000 USD

Schedule

Flexible, with the option to work from Daymark's office in Providence, Rhode Island 5 days a week, and visiting patient's homes as needed.
