Responsibilities

• Holistic Resident Care: Provide dignified assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and specialized feeding.
• Mobility & Safety: Assist with safe resident handling, ambulation, turning, and positioning to ensure skin integrity and physical comfort.
• Vitals & Observation: Serve as the "clinical eyes and ears" for the nursing team; monitor resident conditions and promptly report changes in physical or emotional status.
• Environment Management: Maintain a clean, safe, and organized living space for residents, ensuring their "home" reflects the high standards of our facility.
• Documentation Excellence: Utilize our Electronic Health Record (EHR) system to complete accurate, real-time reports and records in compliance with state and federal guidelines.
• Team Collaboration: Work seamlessly with Unit Managers and the Director of Nursing to implement individualized care plans.

Requirements

• High School Diploma or equivalent.
• Active Wisconsin Certified Nursing Assistant (CNA) certification.
• Compliance with state healthcare worker background check requirements.

Qualifications

• Ability to work effectively in a high-energy, collaborative team environment.
• Basic computer literacy for digital charting.
• A heart for long-term care; previous experience is preferred, but we are happy to train compassionate newcomers.
